Tear Trough & Under-Eye Filler in Singapore
“Tired eyes” can mean very different things.
Some patients have genuine under-eye hollowing. Others have eye bags, pigmentation, skin laxity, or fluid retention. Many have a complex combination of these issues.
This is why I never assume that every dark or tired-looking under-eye automatically needs tear trough filler. Accurate assessment is everything.
What Is the Tear Trough?
The tear trough is the anatomical groove that can appear between the lower eyelid and the upper cheek. When this groove becomes more noticeable, the under-eye area may look:
Shadowed
Deeply sunken
Chronically tired
What Causes Under-Eye Hollowness?
Under-eye hollowing is rarely just a surface-level skin issue. Possible contributors include:
Reduced mid-face (cheek) support
Facial volume loss in the deep fat pads
Age-related tissue thinning and descent
Significant weight loss
Diagnosing the Under-Eye: Not Everything Needs Filler
One of the most important parts of my consultation is distinguishing between different under-eye concerns. A common mistake is trying to use dermal filler to fix a problem it was not designed to treat.
| Under-Eye Concern | Primary Cause | Most Appropriate Treatment Approach |
|---|---|---|
| Tear Trough Hollowing | True structural volume loss | Dermal Fillers (Tear Trough or Mid-Face) |
| Dark Circles | Pigmentation or visible blood vessels | Lasers, topical creams, or Skin Boosters |
| Eye Bags | Fat prolapse (bulging tissue) | Surgical assessment (Blepharoplasty) |
| Puffiness / Fluid | Poor lymphatic drainage | Lifestyle changes, not injectable fillers |
| Crepey Skin | Loss of surface collagen/hydration | Skin Boosters and PN/PDRN |
Tear Trough Hollowing vs Dark Circles & Eye Bags
Tear Trough Hollowing vs Dark Circles
Dark circles are not always caused by volume loss. They may also be related to hyperpigmentation, inherently thin skin, visible underlying blood vessels, or shadowing cast by a bulging eye bag. Filler only addresses selected volume-related concerns. It does not directly remove skin pigmentation or constrict blood vessels.
Tear Trough Hollowing vs Eye Bags
An under-eye hollow and an eye bag are entirely different structural issues. Some patients have a deep groove sitting directly beneath a prominent, bulging eye bag. In these cases, simply filling the hollow does not remove the bag itself and can sometimes make the under-eye look unnatural or puffy.
Why I Assess the Cheeks First
The under-eye region sits directly above the mid-face. Reduced cheek support essentially removes the scaffolding beneath the eyes, which can cause the tear trough to “drop” and appear much more pronounced.
In many selected cases, improving mid-face structural support with cheek filler may be more appropriate—and yield a much more natural result—than placing filler directly into the delicate tear trough.
The cheek and under-eye area must always be assessed together.
When Under-Eye Filler May Be Considered
Tear trough filler may be considered where the main concern is a true volume-related hollow, and the surrounding anatomy provides adequate support.
The objective is usually to create a smoother, seamless transition between the lower eyelid and the cheek, reducing the shadow that causes the eyes to look tired.
When Under-Eye Filler May Not Be Suitable
I am highly cautious, and may advise against tear trough filler, when a patient presents with:
- Significant eye-bag prominence (fat prolapse)
- Persistent under-eye puffiness or fluid retention
- Marked skin laxity (severe sagging)
- Very thin, highly crepey skin
- Predominantly pigment-related dark circles
A High-Precision Treatment
The under-eye is an incredibly delicate and complex anatomical region. It contains vital blood vessels and very thin tissue.
Product choice (selecting the right softness of hyaluronic acid filler), precise amount, exact depth, and placement technique all matter immensely.
This is also an area where adding too much volume is far more noticeable than in other parts of the face, leading to the dreaded “overfilled” look.
A conservative, meticulous treatment plan is therefore non-negotiable.

Frequently Asked Questions
Does tear trough filler hurt?
The under-eye area looks sensitive, but the treatment is generally very well-tolerated. We utilize topical numbing cream prior to the procedure. Furthermore, the specialized dermal fillers we use contain a built-in local anesthetic (lidocaine) to maximize comfort. Many practitioners, including myself, also use a blunt-tipped micro-cannula for this area, which significantly reduces discomfort, bruising, and vascular risk.
Will under-eye filler completely remove my dark circles?
It depends entirely on what is causing the darkness. If your dark circles are actually shadows cast by a deep hollow, filling the hollow will eliminate the shadow and make the area look much brighter. However, if the darkness is caused by brown skin pigmentation (melanin) or purple blood vessels showing through thin skin, filler will not remove that color.
How long does tear trough filler last?
The tear trough is an area of the face with very little mechanical movement compared to the lips or mouth. Because of this, hyaluronic acid fillers break down much more slowly here. Most patients can expect their under-eye filler results to last anywhere from 9 to 18 months, depending on their individual metabolism.
